Electronic Practice ID - Practice QR Code

Dear Parents,

Forgot your patient ID card? Do you need a prescription electronically? Using your public health insurance provider ’s app, you can send us electronic proof of insurance.
This replaces the need to scan your child’selectronic health card💳 at our practice.
To do this, scan the attached Gematik QR code for our practice using your health insurance provider’s app.

You can find this option in your health insurance app under your child’s section, listed as “Certificates,” “Proof of Insurance,” or “Electronic Replacement Certificate (eEB)”—mandatory as of July 2025—or check with your insurance provider if necessary. Your child’s proof of insurance will then be sent electronically right away. The QR code is also posted at the doctor’s office.

You can also do this at any time, even if you haven’t placed a prescription order. We need this certificate as proof of your child’s health insurance coverage for the quarter in which the services were provided. Each quarter begins on January 1, April 1, July 1, and October 1 of each year.
